Who Uses This Form?
- Individuals opening a new Sit Traditional IRA or Roth IRA account
- Sit SEP IRAs are adopted by employers as retirement plans for their employees—only the employer can make contributions

When opening a Sit SEP IRA, please attach the employer’s Form 5305-SEP (see www.irs.gov for more information).
IRA accounts are subject to contribution limits and other rules.
Note: This form is used to open a Sit Traditional or Roth IRA account. To open an non-IRA account, please use the Account Application.
