Who Should Use This Form?
Sit Mutual Funds account owners who would like to grant authority to another person to act on their behalf in all aspects of their account. Please note that trustees may not use this form to grant another person power of attorney for themselves; the trust document provides for trustee succession.
Other Information
The Sit Mutual Funds account owner and the authorized person must sign this form and have it notarized.
