The Affidavit of Domicile is a sworn statement verifying a deceased person’s state of residence at time of death.
Who Uses This Form?
- Personal representative of an estate
- Surviving owner of a joint account
- Beneficiary of an account (if no surviving owners)
Other Information
Affidavit of Domicile must be notarized. For an account with multiple beneficiaries or surviving owners, only one notarized Affidavit of Domicile is required. Death certificates cannot be accepted in place of the Affidavit of Domicile.
