IRA Participants Use This Form:
When you turn 70½, you must begin taking distributions from your Traditional, SEP and SIMPLE IRAs. Sit Mutual Funds will calculate your required minimum distribution upon request. You may request systematic or one-time distributions.

Make sure you complete the federal and state tax-withholding sections.
