IRA Beneficiaries
When the owner of a Sit IRA has died, depending upon who the IRA beneficiary is, certain forms should be completed and returned.
These Forms are Always Required
- An original copy of the death certificate (can be returned upon request)
- A notarized Affidavit of Domicile: Affidavit of Domicile
When the IRA Beneficiary is the Spouse
- This form is required: Spouse Beneficiary IRA Inheritance Request Form
- Complete this form, too, if the IRA beneficiary is a spouse that would like to move some or all of the proceeds into a new Sit IRA: Sit Traditional and Roth IRA Application and Disclosure Statement
When the IRA Beneficiary is Not the Spouse
- This form is required: Non-Spouse, Trust, Estate, or Entity Beneficiary IRA Inheritance Request Form
- Complete this form, too, if the IRA beneficiary is not the spouse and the proceeds are to be moved into a new Sit IRA: Inherited IRA for Non-Spouse, Trust, Estate or Entity Application and Adoption Agreement
