Required Documentation
In order to ensure account security and to comply with government regulations, special documentation is required for the following transactions:
- In the event of the Death of Account Owner, the following requirements apply:
- A new Sit Mutual Funds application must be completed
- A notarized Affidavit of Domicile must accompany the application
- A Medallion Signature Guaranteed letter of instruction written by a surviving owner or executor of the estate must accompany the application and Affidavit of Domicile.
- Authorization of non-owner requirements:
- Notarized Full Trading Authorization form (power of attorney specific to Sit Mutual Funds)
- Account transfer requirements are situation dependent:
- Transfer of non-IRA account from Sit to another firm: Medallion Signature Guaranteed letter of instruction
- Owner/Trustee incapacitation: notarized letter from doctor and Change of Account Information form with Medallion Signature Guarantee
- Beneficiaries:
- If NOT listed on the account at time of death: copy of Letters Testamentary naming Executor of the Estate and Medallion Signature Guaranteed letter of instruction from the Executor
- If no beneficiaries are listed on the account AND the estate qualifies as a “small estate”, a notarized Small Estate Affidavit—check with state’s Department of Revenue
