Sit IRA Transfer/Direct Rollover Form
Description of Form
The Sit IRA Transfer/Direct Rollover Form is used to transfer another IRA into your Sit IRA.
IRA Holders Use This Form To:
- Transfer an existing IRA into a new Sit IRA (must include an IRA application)
- Consolidate your other IRAs (Traditional, SEP, SIMPLE, Roth) into a similar existing Sit IRA
- Move assets from a former employer’s 403(b), 457 or Qualified Plan into your Sit IRA

The custodian of your existing IRA may require a Medallion Signature Guarantee before allowing the transfer. Check with them first.
Your former employer may require that you complete their distribution form instead of the Sit IRA Transfer/Direct Rollover Form. Check with them first.
